So based on the last comment, I searched for the App to make the survey. It worked out pretty okay.

Here are some of examples on what I did, I might decorate a bit to make it fit to my topic.

I will ask them to put their email address so I can send the survey answers to them after the experience. Also will hyperlink the word so it can direct to the AR effect page.

I also tried some of the animated AR effect.

Also tried out on recording gender hate expression. I might combine those audio and the AR effect to give more impactful moment to the audience.

Try out on posters. I tried to use sculptures with different races. Also tried to make it simple as possible so people can get greater impact from the AD effect.
